থার্ড সেফ হলো সেফিক্সিম ট্রাইহাড্রেট ৪০০ মি.গ্রা. সমৃদ্ধ একটি শক্তিশালী এন্টিবায়োটিক ট্যাবলেট। মেডিমেট ফার্মাসিউটিক্যালস লিমিটেড প্রস্তুতকৃত এই ঔষধ মুত্রনালীর সংক্রমণ, টনসিলাইটিস এবং গনেরিয়ার চিকিৎসায় ব্যবহৃত হয়। প্রতি ট্যাবলেটের মূল্য ৳৫০.০০ (৪ টির প্যাক: ৳২০০.০০)।

এই ট্যাবলেট ব্যাকটেরিয়ার কোষ প্রাচীর গঠনে বাধা দিয়ে কাজ করে। বিটা-ল্যাকটামেজ এনজাইম প্রতিরোধী হওয়ায় পেনিসিলিন-প্রতিরোধী জীবাণুর বিরুদ্ধেও কার্যকর। গবেষণায় দেখা গেছে, খাদ্য গ্রহণের সাথে সম্পর্ক없ে ৪০-৫০% শোষণ ক্ষমতা রয়েছে। ই. কোলাই ও স্ট্রেপ্টোকক্কাসের বিরুদ্ধে উচ্চ কার্যকারিতা প্রদর্শন করে।

• অজটিল মুত্রনালীর সংক্রমণ
• কর্ণমধ্যস্থ প্রদাহ ও টনসিলাইটিস
• তীব্র ব্রংকাইটিস
• গনোকক্কাল সংক্রমণ

প্রাপ্তবয়স্কদের জন্য মাত্রা ২০০-৪০০ মিঃগ্রাঃ/দিন। শিশুদের (৬ মাস+) দৈহিক ওজন অনুযায়ী ৮ মিঃগ্রাঃ/কেজি/দিন। ৬ মাসের নিচে ব্যবহার নিষিদ্ধ। পার্শ্বপ্রতিক্রিয়ার মধ্যে ডায়রিয়া, বমি এবং চর্মরাশি উল্লেখযোগ্য। ঔষধটি ৩০ºC তাপমাত্রার নিচে সংরক্ষণ করুন।

ওয়ারফেরিন ব্যবহারকারীদের রক্তজমাট পরীক্ষা আবশ্যক। থার্ড সেফ ২০০ মিঃগ্রাঃ বা সাসপেনশন ফর্মুলেশন বিকল্প হিসেবে বিবেচ্য। গর্ভাবস্থায় ব্যবহারে চিকিৎসকের পরামর্শ জরুরি।

Adults (12+ years): 200-400 mg once or divided twice daily. Uncomplicated gonorrhea: 400 mg single dose. Pediatric (6 months+): 8 mg/kg/day. Treat for 7-14 days. Severe renal impairment (CrCl <20 mL/min): Max 200 mg/day. Take tablets with/without food.

3RD Cef is generally well tolerated. The majority of adverse reactions observed in clinical trials were mild and self limiting in nature. Gastrointestinal disturbances: The most frequent side-effects seen with 3RD Cef are diarrhoea and stool changes; diarrhoea has been more commonly associated with higher doses. Other gastrointestinal side-effects seen less frequently are nausea, abdominal pain, dyspepsia, vomiting and flatulence. Pseudomembraneous colitis has been reported. Central nervous system: headache and dizziness. Hypersensitivity reactions: allergies in the form of rash, pruritis, urticaria, drug fever and arthralgia have been observed. These reactions usually subsided upon discontinuation of therapy. Hematological and clinical chemistry: thrombocytopenia, leukopenia and eosinophilia have been reported. These reactions were infrequent and reversible. Mild transient change in liver and renal function tests have been observed. Miscellaneous: other possible reactions include genital pruritis and vaginitis.

Use during pregnancy only if clearly needed. Lactation safety unknown. Medimet Pharmaceuticals recommends risk-benefit assessment.

Monitor creatinine in chronic kidney disease. Discontinue if persistent diarrhea occurs. Caution in colitis history.

Assess renal function in elderly. Reduce dose in renal impairment. Pediatric dosing by weight/age. No specific hepatic dose adjustment.

Gastric Lavage may be indicated; otherwise, no specific antidote exists. 3RD Cef is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Adverse reactions in small numbers of healthy adult volunteers receiving single doses up to 2 g of 3RD Cef did not differ from the profile seen in patients treated at the recommended doses.
