5-Fluril IV ইনজেকশন বা ইনফিউশন, Techno Drugs Ltd প্রস্তুতকৃত, প্রতি ভায়াল ৳৫৫.০০ মূল্যের এই ফ্লুরোরাসিল (২৫ মিলিগ্রাম/মিলি) সমৃদ্ধ ক্যান্সার চিকিৎসার ঔষধ। মলদ্বার, স্তন, ফুসফুসসহ বিভিন্ন ক্যান্সারে ব্যবহৃত হয়। ঔষধটি ২৫°সে.-এর নিচে অরিজিনাল প্যাকেটে রাখুন—রেফ্রিজারেট ও আলো থেকে দূরে রাখা আবশ্যক।

এটি ডিএনএ সংশ্লেষণে বাধা দিয়ে ক্যান্সার কোষের বিস্তার রোধ করে। দ্রুত ইনজেকশন বা ধীরে ইনফিউশনের মাধ্যমে প্রয়োগ করা হয়। প্রাথমিক মাত্রা রোগীর অবস্থানুসারে ৬-১২ মিগ্রা/কেজি নির্ধারিত হয়। রক্তসংক্রান্ত, হৃদযন্ত্র ও স্নায়বিক পার্শ্বপ্রতিক্রিয়া দেখা দিলে নিবিড় পর্যবেক্ষণ প্রয়োজন।

• সংরক্ষণ: উত্তাপ ও আলো থেকে সুরক্ষিত রাখুন
• এলএসআই কীওয়ার্ড: 5-Fluril ব্যবহার, ফ্লুরোরাসিল বিকল্প, IV ইনজেকশন দাম

সিসপ্লাটিন বা রেডিয়েশনের সাথে যৌথ চিকিৎসায় কার্যকারিতা বৃদ্ধি পায়। সিমেটিডিনের সাথে মিথস্ক্রিয়া ঔষধের মাত্রা বাড়াতে পারে। গর্ভাবস্থায় ব্যবহার নিষিদ্ধ। স্তন্যদানকারী মায়েদের এড়ানো উচিত। ওভারডোজের ক্ষেত্রে লক্ষণভিত্তিক চিকিৎসা প্রয়োজন।

প্রাপ্তবয়স্কদের জন্য প্রাথমিক ডোজ: দিনে ১২-১৩.৫ মিগ্রা/কেজি (৫০০ মিগ্রা/মি২) ৫ দিনের জন্য শিরায় ইনজেকশন, ৪ সপ্তাহ পর পুনরাবৃত্তি। প্যালিয়েটিভ কেয়ারে ৪ দিন ১২ মিগ্রা/কেজি দৈনিক (সর্বোচ্চ ৮০০ মিগ্রা/দিন)। দুর্বল রোগীদের ক্ষেত্রে ৩ দিন ৬ মিগ্রা/কেজি দিয়ে শুরু করতে হবে। IV ইনফিউশন এর জন্য ৫% ডেক্সট্রোজে মিশিয়ে ২-৩ ঘণ্টায় দিতে হবে।

Potentially life-threatening effects: Severe effects from 5-5-Fluril are related to the dosage and duration of therapy.

Cardiac effects: Occasional case reports associating 5-5-Fluril therapy with ‘ischemic cardiac events’ evidence against the autoimmune phenomenon is the fact that in several cases cardiotoxicity occurred within several hours of the first dose.

Hematological effects: Potentially lethal effects caused by severe hematological toxicity may develop within the first 10 days of treatment being instituted but generally resolves within 3 weeks. At the recommended dose and schedule it is rather uncommon for hematological toxicity to be severe. Any ontribute to severe effects from 5-5-Fluril on the blood-forming cells. Thus extensive prior irradiation or the concomitant use of cytotoxic drugs tend to exacerbate the severity of the hematological side effects of 5-5-Fluril.

Neurological effects: Effects on the central nervous system have been occasionally reported and cerebral ataxia is dose-dependent with an incidence of between 3.1 and 7%. Acute cerebellar syndromes and myelopathy have been described following intrathecal 5-5-Fluril. Neurological syndromes may occur rarely after carotid artery perfusion in head and neck cancer. Other effects Allergic reaction (including difficulty in breathing, closing of the throat, swelling of the lips, tongue, or face, or hives), decreased bone marrow function and blood problems (extreme fatigue, easy bruising or bleeding, black, bloody or tarry stools, or fever, chills, or signs of infection), hand-foot syndrome (tingling, pain, redness, swelling or tenderness of the hands and feet), severe vomiting, diarrhoea, frequent bowel movements or watery stools, or sores in the mouth or throat, or stomach pain or heartburn or black, bloody or tarry stools. Other less serious side effects may include mild to moderate nausea, vomiting or loss of appetite, balance problems, confusion, rash and itching, or temporary hair loss. Conjunctivitis, both acute and chronic can proceed to tear duct stenosis and ectropion following prolonged administration. Very chronic administration, extending beyond 3 months, of low dosage has been associated with low systemic toxicity but includes the possibility of painful and tender hands and feet associated with erythema of the extremities.

গর্ভাবস্থায় সম্পূর্ণ নিষিদ্ধ (শ্রেণী ডি)। প্রথম ত্রৈমাসিকে ব্যবহারে ভ্রূণের বিকৃতি ঘটাতে পারে। বুকের দুধে নিঃসৃত হয় কিনা অজানা, তাই স্তন্যদান বন্ধ রাখুন।

রক্তের কাউন্ট সাপ্তাহিক চেক করুন। মুখের প্রথম ঘা দেখা মাত্র চিকিৎসা বন্ধ করুন। ডায়রিয়া বা রক্তপাত হলে তাৎক্ষণিক ব্যবস্থা নিন। ১ গ্রাম/দিনের বেশি ডোজ দেওয়া যাবে না।

শিশুদের জন্য অনুমোদিত নয়। বয়স্ক রোগীদের ওজন অনুযায়ী ডোজ ঠিক করুন। লিভার/কিডনি রোগে ডোজ কমাতে হতে পারে। গর্ভবতী নার্সিং স্টাফকে ঔষধ প্রস্তুত থেকে দূরে রাখুন।

Cases of deliberate overdose are unknown but excessive toxicity because of the hematological effects as described above. There is no specific antidote to 5-5-Fluril toxicity; treatment consists in supportive care. In addition to the hematological toxicity other toxicities will occur with overdose. Sign and symptoms are qualitatively similar to the side effects. Treatment should be performed promptly and appropriate drugs are given to control symptoms of overdose.
