এ-ফেনাক হল ডাইক্লোফেনাক সোডিয়াম ১% ডাব্লিউ/ডাব্লিউ সমৃদ্ধ একটি জেল, প্রস্তুতকারী একমি ল্যাবরেটরিজ লিমিটেড। ৳13.00 মূল্যের এই ঔষধ বাত, পেশী受傷 ও অস্ত্রোপচার-পরবর্তী ফোলা কমায়। আলো থেকে দূরে ৩০°C তাপমাত্রার নিচে সংরক্ষণ করুন।

জেল ফর্মুলেশন স্থানীয়ভাবে কাজ করে বলে পাকস্থলীর জটিলতা কম। প্রধান ব্যবহারের মধ্যে রয়েছে গেঁটেবাত, দাঁতের ব্যথা ও স্ত্রীরোগ সংক্রান্ত প্রদাহ। জেল হিসাবে দিনে ৩-৪ বার প্রয়োগ করুন। তীব্র ব্যথায় ট্যাবলেট বা ইনজেকশনের সাথে যুক্ত করা যায়।

ফার্মাকোলজি: প্রোস্টাগ্ল্যান্ডিন উৎপাদন বন্ধ করে ব্যথা-প্রদাহ নিয়ন্ত্রণ করে। ২ ঘন্টার মধ্যে সর্বোচ্চ রক্ত ঘনত্বে পৌঁছায়। লিভারে বিপাক হয় বলে দেহে জমা হয় না। রক্ত পাতলা করার ঔষধ বা লিথিয়ামের সাথে সতর্কতা প্রয়োজন।

সুরক্ষা: প্রয়োগস্থানে হালকা জ্বালা হতে পারে। গর্ভাবস্থার শেষ trimester এ নিষিদ্ধ। স্তন্যদানকারী মায়েদের চিকিৎসকের পরামর্শ নিতে হবে। একমি ল্যাবরেটরিজের গুণগত মান এ-ফেনাক জেলকে NSAID চিকিৎসায় জনপ্রিয় করে তুলেছে।

Gel: Apply 2-4g 3-4 times daily on affected area. Tablets: 75-150mg/day in 2-3 doses for adults. Reduced dose for elderly/renal impairment. Take after meals. Treatment duration 7-14 days based on condition.

A-Fenac is generally well tolerated. Adverse effects are mild, rare and transient. At the starting of the treatment, however, patients may be sometimes complaining of epigastric pain, eructation, nausea and diarrhea or dizziness or headache. These effects are usually mild in nature. Peripheral edema and skin reactions, such as rash and eczema have also been encountered. A-Fenac Gel may cause local irritation and reddening of the skin and skin rash.

Pregnancy Category C. Contraindicated in 3rd trimester. Compatible with breastfeeding for topical use. Avoid systemic administration during lactation without medical advice.

Monitor renal function in elderly. Avoid ocular/mucosal contact with gel. Regular BP monitoring in hypertensives. Liver function tests required for prolonged use (>2 weeks).

Geriatric: Start with lowest dose. Pediatrics: Not approved <12 years. Renal: Avoid if CrCl <30 mL/min. Hepatic: Contraindicated in severe impairment.
