এ-ফেনাক হলো একমি ল্যাবরেটরিজ লিমিটেড প্রস্তুতকৃত একটি নির্ভরযোগ্য NSAID, যাতে ডাইক্লোফেনাক সোডিয়াম (১২.৫ মি.গ্রা.) সাপোজিটরি আকারে থাকে। ইউনিট মূল্য ৳৯.০০ (প্যাক: ২x৫ মাত্র ৳৯০.০০) সহ এটি প্রদাহজনিত সমস্যায় সাশ্রয়ী সমাধান দেয়। মুখে সেবনের অক্ষম রোগীদের জন্য এই সাপোজিটরি পদ্ধতি পাকস্থলীর জ্বালাপোড়া কমিয়ে কার্যকরী ভূমিকা রাখে।

বাত, অস্ত্রোপচার-পরবর্তী ব্যথা ও স্ত্রীরোগ সংক্রান্ত প্রদাহে এ-ফেনাক প্রোস্টাগ্ল্যান্ডিন উৎপাদন রোধ করে কাজ করে। ঔষধটি দ্রুত শোষিত হয়ে ২ ঘন্টার মধ্যে সর্বোচ্চ প্লাজমা ঘনত্বে পৌঁছায়। সংরক্ষণে শীতল, শুষ্ক স্থান (৩০°সে.-এর নিচে) প্রয়োজন। প্রাথমিকভাবে হালকা পেটের গোলযোগ দেখা দিলেও চিকিৎসা চালিয়ে গেলে সমস্যা কমে।

মাত্রানুযায়ী প্রাপ্তবয়স্কদের জন্য ৫০ মি.গ্রা. সাপোজিটরি দিনে ২-৩ বার (সর্বোচ্চ ১৫০ মি.গ্রা.) ব্যবহার্য। বিশেষ রোগীদের ক্ষেত্রে মাত্রা সমন্বয় করা আবশ্যক। লিথিয়াম, অ্যান্টিকোয়াগুল্যান্টস ও মেথোট্রেক্সেটের সাথে ইন্টারঅ্যাকশন ঘটতে পারে। গর্ভাবস্থায় অপরিহার্য না হলে ব্যবহার না করার পরামর্শ দেওয়া হয়।

একমি ল্যাবরেটরিজের অন্যান্য ডাইক্লোফেনাক প্রোডাক্ট যেমন ২৫/৫০ মি.গ্রা. ট্যাবলেট ও ১% জেলের পাশাপাশি এ-ফেনাক বাত ও প্রদাহ নিয়ন্ত্রণে গুরুত্বপূর্ণ ভূমিকা রাখে। বিকল্প হিসেবে অন্যান্য ডাইক্লোফেনাক ব্র্যান্ড সম্পর্কে জানুন।

Dosage & Administration

Suppository: Adults 50mg 2-3 times daily (Max 150mg/day). Pediatric dose 1-3mg/kg. Dose reduction required for elderly and renal impaired patients. Follow proper suppository administration techniques.

Side Effects

A-Fenac is generally well tolerated. Adverse effects are mild, rare and transient. At the starting of the treatment, however, patients may be sometimes complaining of epigastric pain, eructation, nausea and diarrhea or dizziness or headache. These effects are usually mild in nature. Peripheral edema and skin reactions, such as rash and eczema have also been encountered. A-Fenac Gel may cause local irritation and reddening of the skin and skin rash.

Pregnancy & Lactation

Pregnancy Category C (D in third trimester). Low concentration in breast milk. Use lowest effective dose for shortest duration during lactation.

Precautions & Warnings

Monitor blood pressure in hypertensive patients. Regular renal/hepatic function tests needed during prolonged therapy. Avoid in patients with IBD or coagulation disorders.

Use in Special Populations

Pediatric: Only for children >1 year. Geriatric: Start with 50% reduced dose. Renal impairment: Avoid if creatinine clearance <30 mL/min.
