অ্যাবাক্টেন পাউডার ফর সাসপেনশন হল এজিথ্রোমাইসিন ডাইহাইড্রেট (২০০ মিগ্রা/৫ মিলি) সমৃদ্ধ একটি ম্যাক্রোলাইড অ্যান্টিবায়োটিক, যা আর্জেস লাইফ সাইন্স লিমিটেড দ্বারা উৎপাদিত। ৳১৩০.০০ মূল্যের এই পাউডার ফর সাসপেনশন ব্রংকাইটিস, নিউমোনিয়া, ত্বকের সংক্রমণ এবং টাইফয়েড জ্বরের চিকিৎসায় ব্যবহৃত হয়। এটি ব্যাকটেরিয়ার প্রোটিন সংশ্লেষণে বাধা দিয়ে কাজ করে।

মাত্রা বয়স ও রোগের ধরন অনুযায়ী পরিবর্তিত হয়। প্রাপ্তবয়স্কদের জন্য ৩ দিন দিনে একবার ৫০০ মিগ্রা, আর শিশুদের (১৫-৪৫ কেজি) ২০০-৪০০ মিগ্রা/দিন দেওয়া হয়। টাইফয়েড জ্বরে শিশুদের ৭-১০ দিন প্রতি দিন ৫০০ মিগ্রা প্রয়োজন। ঔষধটি শুষ্ক, আলো ও তাপ থেকে দূরে সংরক্ষণ করুন।

মুখ্য সংকেতসমূহের মধ্যে রয়েছে শ্বাসনালীর সংক্রমণ, মধ্যকর্ণের প্রদাহ এবং ক্ল্যামাইডিয়া সংক্রমণ। বমি বা ডায়রিয়ার মতো হালকা পার্শ্বপ্রতিক্রিয়া দেখা দিতে পারে। আরগোট এলকালয়েডের সাথে ব্যবহার এড়িয়ে চলুন এবং সাইক্লোস্পোরিন ব্যবহারকারীদের মনিটরিং করুন।

প্রেগন্যান্সি ক্যাটাগরি বি শ্রেণিভুক্ত এই ঔষধ শুধুমাত্র প্রয়োজন হলে ব্যবহার করুন। স্তন্যদানকালে সতর্কতা অবলম্বন করুন। অতidosডোজের ক্ষেত্রে গ্যাস্ট্রিক লেভেজের পরামর্শ দেওয়া হয়। বিকল্প হিসেবে অ্যাবাক্টেন ৫০০ মিগ্রা বিবেচনা করা যেতে পারে।

Adults: 500mg for 3 days or 500mg day 1 + 250mg next 4 days. Children (15-25kg): 200mg daily × 3 days. Typhoid: 500mg × 7-10 days. Take suspension 1hr before/after meals. Dose adjustment needed in renal impairment.

Abacten is well tolerated with a low incidence of side-effects. Most side-effects observed were mild to moderate in severity. The majority of side-effects were gastrointestinal in origin with nauseas, abdominal discomfort (pain/cramps), vomiting, flatulence, diarrhoea and loose stools being occasionally observed. Allergic reactions such as rash or photosensitivity have occurred and there have also been rare reports of serious hypersensitivity reactions. Reversible elevations in liver transaminases have been seen with a frequency similar to the comparative macrolides and penicillins used in clinical trials. Rarely, cases of cholestatic jaundice have been observed. Transient mild reductions in neutrofil counts have occasionally been observed in clinical trials, although a causal relationship to azithromycin has not been established. Hearing impairment: In investigational studies where higher doses were used for prolonged periods of time, reversible hearing impairment was seen in some patients.

Pregnancy Category B – use only if clearly needed. Breastfeeding safety unknown. Consider risk-benefit ratio. Consult physician before use.

Monitor for superinfections. Renal function tests required in kidney disease. ECG monitoring in cardiac patients. Complete full course to prevent resistance.

Pediatric: Safe above 6 months. Geriatric: Monitor hepatic/renal function. Renal impairment: Adjust dose if CrCl <40. Hepatic disease: Contraindicated.

There is no data on overdosage with Abacten. Typical symptoms of overdosage with macrolide antibiotics include hearing loss, severe nausea, vomiting and diarrhoea. Gastric lavage and general supportive measures are indicated.
