অ্যাবিরাক্সেন ট্যাবলেট হল ২৫০ মিগ্রা অ্যাবিরাটেরোন অ্যাসিটেট সমৃদ্ধ একটি ক্যান্সার চিকিত্সার ঔষধ। এভারেষ্ট ফার্মাসিউটিক্যালস লিমিটেড প্রস্তুতকৃত এই ট্যাবলেট-এর ইউনিট মূল্য ৳৫৮০ এবং ২৮ ট্যাবলেটের প্যাক মূল্য ৳১৬,২৪০। ঔষধটি ২৫°সে.-এর নিচে সংরক্ষণ করতে হবে।

এই ঔষধ CYP17 এনজাইম প্রতিরোধ করে প্রোস্টেট ক্যান্সার কোষের অ্যান্ড্রোজেন উৎপাদন বন্ধ করে। এটি মেটাস্ট্যাটিক ক্যাস্ট্রেশন-প্রতিরোধী (CRPC) এবং উচ্চ-ঝুঁকিপূর্ণ কাস্ট্রেশন-সংবেদনশীল প্রোস্টেট ক্যান্সার (CSPC)-এ প্রেডনিসোনের সাথে ব্যবহৃত হয়। খালি পেটে (খাবারের ১ ঘণ্টা আগে বা ২ ঘণ্টা পরে) সেব্য।

প্রস্তাবিত ডোজ হল দৈনিক ১,০০০ মিগ্রা (৪টি ২৫০ মিগ্রা ট্যাবলেট) প্রেডনিসোনের সাথে। যকৃতের সমস্যায় ডোজ কমিয়ে ২৫০ মিগ্রা করা হয়। CYP3A4 ইন্ডিউসার ব্যবহারে ডোজ সামঞ্জস্য প্রয়োজন। সাধারণ পার্শ্বপ্রতিক্রিয়ার মধ্যে ক্লান্তি, উচ্চ রক্তচাপ এবং হাইপোক্যালেমিয়া রয়েছে।

শিশুদের জন্য অ্যাবিরাক্সেনের নিরাপত্তা নির্ণয় হয়নি। গর্ভাবস্থায় এটি ভ্রূণের ক্ষতি করতে পারে। CYP2D6/CYP2C8 সাবস্ট্রেটের সাথে মিথস্ক্রিয়া সতর্কতা প্রয়োজন। অতিরিক্ত মাত্রায় সেবনে即时 ঔষধ বন্ধ করে চিকিত্সা নিতে হবে।

Dosage & Administration

Adults: 1,000 mg (four 250 mg tablets) once daily on empty stomach. Take with 5 mg prednisone twice (CRPC) or once daily (CSPC). Reduce to 250 mg for moderate hepatic impairment. Must combine with GnRH analog therapy. Swallow tablets whole; avoid crushing.

Side Effects

The most common adverse reactions are fatigue, arthalgia, hypertension, nausea, edema, hypokalemia, hot flush, diarrhea, vomiting, upper respiratory infection, cough, and headache.

Pregnancy & Lactation

Pregnancy Category X. Contraindicated due to fetal risk. Breastfeeding not recommended.

Precautions & Warnings

Monitor BP, potassium, LFTs monthly. Manage edema with diuretics. Store below 25°C protected from light.

Use in Special Populations

No dose adjustment for elderly/renal impairment. Pediatric use not approved. Hepatic impairment requires dose reduction.

Overdose Effects

There is no specific antidote. In the event of an overdose, Abiraxen should be stopped, general supportive measures are undertaken, including monitoring for arrhythmias and cardiac failure and assess liver function.
