এসিকার্ড (র‍্যামিপ্রিল ১.২৫ মি.গ্রা.) হল হেলথকেয়ার ফার্মাসিউটিক্যালস লিমিটেড প্রস্তুতকৃত একটি এসিই ইনহিবিটর ট্যাবলেট। প্রতি ইউনিট মূল্য ৳৩.০০, স্ট্রিপ ৳৩০.০০ এবং ৩×১০ প্যাক ৳৯০.০০। আলো থেকে দূরে ৩০°C তাপমাত্রায় সংরক্ষণ করুন।

এই র‍্যামিপ্রিল ঔষধ উচ্চ রক্তচাপ, হার্ট ফেইলিওর ও কার্ডিওভাস্কুলার ঝুঁকি হ্রাস করে। মায়োকার্ডিয়াল ইনফার্কশন পরবর্তী চিকিৎসায় কার্যকর। ডোজ ১.২৫ মি.গ্রা. থেকে ২০ মি.গ্রা. পর্যন্ত নিয়ন্ত্রিত হয়। বৃক্কের সমস্যায় ডোজ সামঞ্জস্য প্রয়োজন।

ডায়াবেটিক নেফ্রোপ্যাথি ও পেরিফেরাল ভাস্কুলার ডিজিজে ব্যবহারযোগ্য। এনএসএআইডি ওষুধ ও পটাশিয়াম স্পায়ারিং ডায়ইউরেটিকস এড়িয়ে চলুন। সাধারণ পার্শ্বপ্রতিক্রিয়ার মধ্যে মাথা ঘোরা ও শুষ্ক কাশি উল্লেখযোগ্য। গর্ভাবস্থায় নিষিদ্ধ।

চিকিৎসকের পরামর্শে দীর্ঘমেয়াদী ব্যবহারে কার্ডিওভাস্কুলার সুরক্ষা নিশ্চিত হয়। এসিকার্ডের বিকল্প হিসেবে এসিকার্ড ২.৫ মি.গ্রা. ব্যবহার করা যেতে পারে। ঔষধ সেবনকালে নিয়মিত রক্তচাপ মনিটরিং আবশ্যক।

Dosage & Administration

Hypertension: Initial 1.25-2.5 mg daily, max 20 mg. Heart failure: 2.5 mg twice daily. Renal impairment: Start with 1.25 mg. Geriatric patients: Begin with 1.25 mg. Swallow tablet whole with water. Adjust dose every 2 weeks.

Side Effects

Acecard is generally well tolerated. Dizziness, headache, fatigue and asthenia are commonly reported side effects. Other side effects occurring less frequently include symptomatic hypotension, cough, nausea, vomiting, diarrhoea, rash, urticaria, oliguria, anxiety, amnesia etc. Angioneurotic oedema, anaphylactic reactions and hyperkalaemia have also been reported rarely.

Pregnancy & Lactation

Pregnancy Category D. Contraindicated in 2nd/3rd trimesters. Breastfeeding not recommended. May cause fetal renal damage.

Precautions & Warnings

Monitor serum potassium and creatinine levels. Avoid dehydration. Stop before major surgery. Low-salt diet recommended. Caution in collagen vascular diseases.

Use in Special Populations

Geriatric: Start with 1.25 mg. Renal impairment: Adjust dose if CrCl <40 mL/min. Hepatic disease: Max 2.5 mg/day. Pediatrics: Safety not established.

Overdose Effects

Sign and symptom: Overdosage may cause excessive peripheral vasodilatation (with marked hypotension, shock), bradycardia, electrolyte disturbances, and renal failure.

Management: Primary detoxifcation by, for example, gastric lavage, administration of adsorbents, sodium sulfate; (if possible during the frst 30 minutes). In the event of hypotension administration of α1-adrenergic agonists (e.g. norepinephrine, dopamine) or angiotensin II (angiotensinamide), which is usually available only in scattered research laboratories, must be considered in addition to volume and salt substitution.
