এসিকার্ড (র‍্যামিপ্রিল ২.৫ মি.গ্রা.) হেলথকেয়ার ফার্মাসিউটিক্যালস লিমিটেড প্রস্তুতকৃত একটি এসিই ইনহিবিটর। উচ্চ রক্তচাপ, হার্ট ফেইলিউর ও কার্ডিওভাস্কুলার ঝুঁকি নিয়ন্ত্রণে এটি ব্যবহৃত হয়। প্রতি ট্যাবলেটের মূল্য ৳৫.০০ (স্ট্রিপ: ৳৫০.০০) সহ সাশ্রয়ী চিকিৎসা প্রদান করে। দীর্ঘমেয়াদী ব্যবহারের জন্য উপযোগী এই ট্যাবলেট দিনে এক বা দুবার সেব্য।

ওষুধটি র‍্যামিপ্রিল এর মাধ্যমে এনজিওটেনসিন II গঠন রোধ করে রক্তনালী প্রসারিত করে।

উচ্চ রক্তচাপ: দৈনিক ১.২৫–২.৫ মি.গ্রা. শুরুতে
হার্ট ফেইলিউর: দিনে দুবার ২.৫ মি.গ্রা.
বৃক্কের রোগ: দৈনিক ১.২৫ মি.গ্রা.

৩০°C এর নিচে আলো থেকে দূরে সংরক্ষণ করুন। সাধারণ পার্শ্বপ্রতিক্রিয়ার মধ্যে মাথা ঘোরা (৯%), শুষ্ক কাশি (৩%) রয়েছে। গর্ভাবস্থায় ব্যবহার নিষিদ্ধ। ৳১৫০.০০ মূল্যের প্যাক (৩×১০ ট্যাবলেট) এক মাসের চাহিদা পূরণ করে। ট্যাবলেট আকারে প্রাপ্ত এই ঔষধ দীর্ঘমেয়াদী ব্যবহারে নিরাপদ।

Dosage & Administration

Initial dose 1.25-2.5 mg daily for hypertension. Max 20 mg/day. Renal impairment: Start with 1.25 mg. Post-MI: 2.5 mg twice daily. Swallow tablet whole. Treatment duration may continue lifelong based on condition.

Side Effects

Acecard is generally well tolerated. Dizziness, headache, fatigue and asthenia are commonly reported side effects. Other side effects occurring less frequently include symptomatic hypotension, cough, nausea, vomiting, diarrhoea, rash, urticaria, oliguria, anxiety, amnesia etc. Angioneurotic oedema, anaphylactic reactions and hyperkalaemia have also been reported rarely.

Pregnancy & Lactation

Pregnancy category D. Fetal harm in 2nd/3rd trimesters. Contraindicated in lactation. Consult doctor before conception.

Precautions & Warnings

Monitor serum potassium regularly. Avoid dehydration. Discontinue 3 days pre-surgery. Renal function tests mandatory. Use quality assured medicines from Healthcare Pharmaceuticals.

Use in Special Populations

Elderly: Start with 1.25 mg. Renal disease: Reduce dose if CrCl <40 mL/min. Hepatic impairment: Max 2.5 mg/day. Not approved for pediatric use.

Overdose Effects

Sign and symptom: Overdosage may cause excessive peripheral vasodilatation (with marked hypotension, shock), bradycardia, electrolyte disturbances, and renal failure.

Management: Primary detoxifcation by, for example, gastric lavage, administration of adsorbents, sodium sulfate; (if possible during the frst 30 minutes). In the event of hypotension administration of α1-adrenergic agonists (e.g. norepinephrine, dopamine) or angiotensin II (angiotensinamide), which is usually available only in scattered research laboratories, must be considered in addition to volume and salt substitution.
