এসিকার্ড ৫ মি.গ্রা. ট্যাবলেট হলো র‍্যামিপ্রিল সমৃদ্ধ একটি এন্টিহাইপারটেনসিভ ওষুধ, যা উত্পাদন করে হেলথকেয়ার ফার্মাসিউটিক্যালস লিমিটেড। এই ট্যাবলেট উচ্চ রক্তচাপ, হার্ট ফেইলিউর এবং কার্ডিওভাস্কুলার ঝুঁকি হ্রাসে ব্যবহৃত হয়। প্রতি স্ট্রিপে ১০টি ট্যাবলেট রয়েছে, দাম ৳৮০.০০ (প্রতিটি ৳৮.০০) এবং ৩ স্ট্রিপের প্যাকের মূল্য ৳২৪০.০০।

এসিএ ইনহিবিটর হিসেবে র‍্যামিপ্রিল রক্তনালী প্রসারিত করে রক্তচাপ নিয়ন্ত্রণ করে। মায়োকার্ডিয়াল ইনফার্কশন-পরবর্তী চিকিৎসা ও ডায়াবেটিক নেফ্রোপ্যাথিতে এর ভূমিকা উল্লেখযোগ্য। করোনারি আর্টারি ডিজিজ বা পেরিফেরাল ভাস্কুলার ডিজিজযুক্ত রোগীদের কার্ডিওভাস্কুলার মৃত্যুর ঝুঁকি কমাতেও এটি সহায়ক।

ডোজ নির্দেশনা: উচ্চরক্তচাপে প্রাথমিক ডোজ ১.২৫–২.৫ মি.গ্রা./দিন। হার্ট ফেইলিউরে ইনফার্কশনের ২ দিন পর ২.৫ মি.গ্রা. দিনে দুইবার দেওয়া হয়। বৃক্কের অসুখে ডোজ সামঞ্জস্য প্রয়োজন। পার্শ্বপ্রতিক্রিয়ার মধ্যে মাথা ঝিমঝিম বা কাশি সাধারণ, তবে এনজিওইডিমা খুবই বিরল। ৩০°C-এ সংরক্ষণ করুন এবং আলো থেকে দূরে রাখুন।

মূত্রবর্ধক বা ব্যথানাশকের সাথে ব্যবহার সতর্কতা প্রয়োজন। গর্ভাবস্থায় এড়িয়ে চলুন। নিয়মিত রক্তচাপ পরিমাপের মাধ্যমে চিকিৎসা ফলপ্রসূ করা যায়। এসিকার্ড ১.২৫ মি.গ্রা. বা এসিকার্ড ২.৫ মি.গ্রা. বিকল্প হিসেবে বিবেচনা করা যেতে পারে।

Dosage & Administration

Initial adult dose 1.25-2.5 mg daily, titrate up to 20 mg maximum. Renal impairment patients start with 1.25 mg. Halve dose for elderly. Swallow tablet whole with water. Continue as long as prescribed by physician for chronic conditions.

Side Effects

Acecard is generally well tolerated. Dizziness, headache, fatigue and asthenia are commonly reported side effects. Other side effects occurring less frequently include symptomatic hypotension, cough, nausea, vomiting, diarrhoea, rash, urticaria, oliguria, anxiety, amnesia etc. Angioneurotic oedema, anaphylactic reactions and hyperkalaemia have also been reported rarely.

Pregnancy & Lactation

Pregnancy Category D – fetal toxicity risk. Contraindicated in lactation. Discontinue immediately if pregnancy detected. Pre-pregnancy counseling mandatory.

Precautions & Warnings

Monitor serum potassium monthly. Renal function tests every 3 months. Report salt depletion symptoms. Discontinue before major surgeries requiring anesthesia.

Use in Special Populations

Geriatric: Start with 1.25 mg. Renal dosing based on creatinine clearance. Hepatic impairment max 2.5 mg/day. Not approved for pediatric use.

Overdose Effects

Sign and symptom: Overdosage may cause excessive peripheral vasodilatation (with marked hypotension, shock), bradycardia, electrolyte disturbances, and renal failure.

Management: Primary detoxifcation by, for example, gastric lavage, administration of adsorbents, sodium sulfate; (if possible during the frst 30 minutes). In the event of hypotension administration of α1-adrenergic agonists (e.g. norepinephrine, dopamine) or angiotensin II (angiotensinamide), which is usually available only in scattered research laboratories, must be considered in addition to volume and salt substitution.
