এসিফিক্স (র‌্যাবিপ্রাজল সোডিয়াম) হলো বেক্সিমকো ফার্মাসিউটিক্যালস প্রস্তুতকৃত একটি প্রোটন পাম্প ইনহিবিটর। ২০ মিগ্রা শক্তির এই গ্যাস্ট্রো-রেজিস্ট্যান্ট ট্যাবলেট ডিওডেনাল আলসার, জিইআরডি, এবং জলিনজার-এলিসন সিনড্রোমের চিকিৎসায় ব্যবহৃত হয়। প্রতি ট্যাবলেটের দাম ৳৭.০০ এবং ১০ ট্যাবলেটের স্ট্রিপ মূল্য ৳৭০.০০।

ওষুধটি পাকস্থলির প্যারাইটাল কোষে H⁺/K⁺-ATPase এনজাইম বন্ধ করে এসিড নিঃসরণ নিয়ন্ত্রণ করে। র‌্যাবিপ্রাজল সোডিয়াম ভিত্তিক এই এন্টেরিক কোটেড ট্যাবলেট ৪-৮ সপ্তাহের মধ্যে ৯০% রোগীর আলসার নিরাময় করে। বাংলাদেশে সহজলভ্যতার কারণে এটি জনপ্রিয় অ্যান্টি-আলসার ওষুধ।

কিটোকোনাজল ও এটাজানাভিরের সাথে মিথস্ক্রিয়া দেখা দিতে পারে। ৩% রোগীর মধ্যে মাথাব্যথা বা ডায়রিয়া হয়। গর্ভাবস্থায় সতর্কতার সাথে ব্যবহার করুন (এফডিএ ক্যাটাগরি সি)। ৩০°C এর নিচে, আলো ও আর্দ্রতা থেকে দূরে সংরক্ষণ করুন।

Dosage & Administration

20 mg once daily for 4-8 weeks in duodenal ulcers. GERD: 20 mg for 4-8 weeks. Pediatric dose: 5 mg (<15 kg) or 10 mg (≥15 kg) for 12 weeks. Zollinger-Ellison: 60-120 mg/day. Swallow enteric-coated tablet whole.

Side Effects

In general, Acifix is well-tolerated in both short-term and long-term studies. Acifix may sometimes cause headache, diarrhoea, abdominal pain, vomiting, constipation, dry mouth, increased or decreased appetite, muscle pain, drowsiness, dizziness.

Pregnancy & Lactation

Pregnancy Category C: No risk in animal studies,但 human data limited. Excreted in breast milk. Use Beximco product cautiously per physician advice.

Precautions & Warnings

Supplement calcium/vitamin B12 during long-term use. Monitor bone density. Avoid >1 year continuous use. Rule out malignancy via endoscopy in GERD patients.

Use in Special Populations

Pediatric use only for GERD in 1-11 years. No dose adjustment in renal/hepatic impairment. Monitor fracture risk in elderly.

Overdose Effects

The maximum established exposure has not exceeded 60 mg twice daily, or 160 mg once daily. Effects are generally minimal, representative of the known adverse event profile and reversible without further medical intervention. No specific antidote is known. Acifix is extensively protein bound and is, therefore, not dialysable. As in any case of overdose, treatment should be symptomatic and general supportive measures should be utilised.
