এসিফিন হলো সেফট্রায়াক্সন সোডিয়াম সমৃদ্ধ তৃতীয় জেনারেশনের শক্তিশালী এন্টিবায়োটিক। এসিআই লিমিটেড প্রস্তুতকৃত এই ইঞ্জেকশন (২৫০ মি.গ্রা./ভায়াল) নিউমোনিয়া, মেনিনজাইটিস ও গনোরিয়ার চিকিৎসায় ব্যবহৃত হয়। প্রতি ভায়ালের মূল্য ৳১০০.৩০ টাকা।

এসিফিন ব্যাকটেরিয়ার কোষ প্রাচীর গঠনে বাধা দিয়ে বিটা-ল্যাকটামেজ এনজাইম প্রতিরোধী। ৬-৯ ঘণ্টা হাফ-লাইফের কারণে দিনে একবার ব্যবহার যথেষ্ট। ঔষধের ৪০-৬৫% মূত্রের মাধ্যমে ও বাকিটি পিত্তে নিষ্কাশিত হয়। আলো ও আর্দ্রতা থেকে ৩০°C তাপমাত্রায় সংরক্ষণ করুন।

প্রাপ্তবয়স্কদের ডোজ ১-২ গ্রাম/দিন, শিশুদের ৫০-৭৫ মি.গ্রা./কেজি। সার্জিক্যাল প্রোফাইল্যাক্সিসে ১ গ্রাম IV প্রয়োগ করুন।

পার্শ্বপ্রতিক্রিয়ার মধ্যে ডায়রিয়া, চর্মপ্রদাহ ও লিভার এনজাইম বৃদ্ধি দেখা যায়। গর্ভাবস্থায় সতর্কতা অবলম্বন করুন। বিকল্প হিসেবে আইএম ইঞ্জেকশন এর অন্যান্য সেফালোস্পোরিন বিবেচনা করুন।

Dosage & Administration

Adults: 1-2g daily IV/IM (max 4g). Pediatrics: 50-75mg/kg/day. Meningitis dose: 100mg/kg. Treatment duration typically 4-14 days. Administer IV injections over 2-4 minutes.

Side Effects

Aciphin is generally well tolerated. A few side effects such as gastro-intestinal effects including diarrhea, nausea and vomiting, stomatitis and glossitis; cutaneous reactions including rash, pruritus, urticaria, edema and erythema multiforme; hematologic reactions including eosinophilia, thrombocytopenia, leucopenia, anemia and neutropenia; hepatic reactions including elevations of SGOT or SGPT, bilirubinemia; CNS reactions including nervousness, confusion, sleep disturbances, headache, hyperactivity, convulsion, hypertonia and dizziness were reported. Local phlebitis occurs rarely following intravenous administration but can be minimized by slow injections over 2-4 minutes.

Pregnancy & Lactation

Pregnancy Category B. Caution in lactation – excreted in breast milk.

Precautions & Warnings

Keep epinephrine ready for anaphylaxis. Monitor gallbladder sludge with prolonged use. Regular blood tests recommended.

Use in Special Populations

Pediatrics: Safe above 1 month. Elderly: Monitor renal function. Hepatic impairment: Dose adjustment needed.

Overdose Effects

There is no specific antidote. Treatment of overdosage should be symptomatic.
