Actifol Tablet 15 mg

Actifol Tablet is a pharmaceutical preparation containing Folinic Acid (15 mg) manufactured by ACI Limited. As a calcium folinate hydrate formulation, it counteracts folate antagonist toxicity and treats megaloblastic anemia. Priced at ৳25.00 per unit, it comes in strips of 10 tablets (৳250.00) and packs of 3×10 (৳750.00). Store below 30°C, protected from light.

This tablet neutralizes toxic effects of methotrexate and other folate antagonists. It acts as cofactor in nucleic acid synthesis, addressing DNA replication errors in folate deficiency. Particularly effective in “folinate rescue” therapy, it protects healthy cells during high-dose methotrexate treatments while maintaining chemotherapeutic efficacy against cancer cells.

Methotrexate rescue: 10 mg/m² every 6 hours (max 10 doses). Megaloblastic anemia: 5-15 mg daily orally/IV. Initiate within 24-48 hours post-methotrexate, continue 7-10 days. Reduce dose by 50% in renal impairment. Administer with alkaline fluids to enhance solubility.

The most common side effects of Actifol are mucositis, stomatitis, leukopenia and/or diarrhea, which may be dose-limiting.

Pregnancy Category C. Use only if benefits outweigh risks. Excretion in breast milk unknown. Monitor infants during lactation. Consider pumping/dumping for 72 hours post-dose.

Requires hematologist supervision. Mandatory for methotrexate doses >500 mg/m². Monitor serum creatinine and methotrexate levels every 12-24 hours. Maintain urinary pH >7 with bicarbonate hydration. Discontinue if neurologic symptoms appear.

Pediatric safety unestablished. Geriatric patients require renal function assessment. Reduce dose by 25% in hepatic impairment. Dialysis patients need 50% supplemental dose post-procedure.

There have been no reported sequelae in patients who have received significantly more calcium folinate than the recommended dosage. Flowever, excessive amounts of calcium folinate may nullity the chemotherapeutic effect of folic acid antagonists. There is no specific antidote to calcium folinate overdose. In cases of overdosage patients should be given appropriate supportive care.
