Montril (Montelukast Sodium) is a leukotriene receptor antagonist manufactured by Aristopharma Ltd. Available as 5 mg chewable tablets, it effectively manages asthma, exercise-induced bronchoconstriction, and allergic rhinitis. Priced at ৳8.00 per unit, it offers cost-effective therapy with strips of 10 tablets (৳80.00) and packs of 30 tablets (৳240.00).

This medication blocks cysteinyl leukotriene receptors, reducing inflammation and bronchoconstriction. Clinical studies show efficacy in preventing asthma attacks and seasonal allergy symptoms. Its once-daily dosage (5-10 mg based on age) ensures patient compliance. Storage requires a cool, dry place (<30°C), protected from light and moisture.

Safety Profile: Common side effects include headache (18% cases), nausea, and diarrhea. Rare risks involve neuropsychiatric symptoms like agitation or suicidal thoughts. Avoid with potent CYP450 inducers. Pregnant/lactating women should use caution despite low systemic absorption.

Common: Diarrhoea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection.

Uncommon: Akathisia, anxiety, arthralgia, asthenia, abnormal behavior, depression, dizziness, drowsiness, dry mouth, haemorrhage, irritability, malaise, muscle complaints, oedema, seizure, abnormal sensation, sleep disorders.

Rare: Angioedema, concentration impaired, disorientation, eosinophilic granulomatosis with polyangiitis, erythema nodosum, hallucination, hepatic disorders, memory loss, palpitations, pulmonary eosinophilia, suicidal tendencies, tremor.

Pregnancy Category B. Excreted in breast milk (1:50 plasma ratio). Use during lactation only if benefits outweigh risks.

Monitor for depression/aggression. Not substitutable for inhaled corticosteroids. Avoid abrupt steroid withdrawal. CNS effects may impair driving.

No pediatric hepatic dose adjustment. Geriatric patients show similar pharmacokinetics. Safety in children <6mo not established.

There were no adverse experiences in the majority of overdosage reports. The most frequently occurring adverse experiences were consistent with the safety profile of Montril and included abdominal pain, somnolence, thirst, headache, vomiting and psychomotor hyperactivity. In the event of overdose, it is reasonable to employ the usual supportive measures; e.g., remove unabsorbed material from the gastrointestinal tract, employ clinical monitoring, and institute supportive therapy, if required.
