Monuril Oral Powder 3 gm/sachet

Monuril is an intracellular antibiotic containing Fosfomycin Trometamol (3g/sachet) as its active ingredient. Manufactured by UniMed UniHealth Pharmaceuticals Ltd, this oral powder treats uncomplicated urinary tract infections (acute cystitis) in adult women caused by Escherichia coli and Enterococcus faecalis. Priced at ৳350.00 per sachet, it offers a single-dose regimen for quick relief.

The Fosfomycin Trometamol in Monuril disrupts bacterial cell wall synthesis by inhibiting enolpyruvyl transferase, preventing pathogens from multiplying. Its broad-spectrum activity covers both gram-positive and gram-negative aerobic bacteria. As an oral powder, it ensures rapid absorption and high urinary concentration within 2–4 hours.

Adults take one sachet dissolved in 100ml water. Avoid using medications like metoclopramide that reduce its efficacy. Common side effects include diarrhea (10.4%), headache (10.3%), and nausea (5.2%). Store below 25°C, away from light, and keep out of children’s reach.

Safety Note: Not recommended for children or pregnant/lactating women unless benefits outweigh risks. Overdose may cause vertigo or hearing issues. Consult doctors before combining with motility-enhancing drugs.

In clinical trials, the most frequently reported adverse events occurring in >1% of the study population regardless of drug relationship were: diarrhea 10.4%, headache 10.3%, vaginitis 7.6%, nausea 5.2%, rhinitis 4.5%, back pain 3.0%, dysmenorrhea 2.6%, pharyngitis 2.5%, dizziness 2.3%, abdominal pain 2.2%, pain 2.2%, dyspepsia 1.8%, asthenia 1.7%, and rash 1.4%.The following adverse events occurred in clinical trials at a rate of less than 1%, regardless of drug relationship: abnormal stools, anorexia, constipation, dry mouth, dysuria, ear disorder, fever, flatulence, flu syndrome, hematuria, infection, insomnia, lymphadenopathy, menstrual disorder, migraine, myalgia, nervousness, paresthesia, pruritus, SGPT increased, skin disorder, somnolence, and vomiting.

Use during pregnancy only if clearly needed (Category B). Consult doctor while breastfeeding. Limited data on milk excretion.

Avoid multiple doses to prevent antibiotic resistance. Report prolonged diarrhea. May affect urine glucose tests in diabetics.

Pediatric use contraindicated. Monitor renal function in elderly. Dose reduction in renal impairment. No hepatic dosage adjustment needed.

When Monuril is coadministered with metoclopramide, which increases gastrointestinal motility, lowers the serum concentration and urinary excretion of fosfomycin. Other drugs that increase gastrointestinal motility may produce similar effects.
