Motigut Oral Suspension 5 mg/5 ml

Motigut is a gastrointestinal medication containing Domperidone Maleate (5 mg/5 ml) as an oral suspension. Manufactured by Square Pharmaceuticals PLC, it regulates gastric motility and alleviates nausea. Priced at ৳40.00 per unit, this oral suspension requires storage below 30°C, protected from light and moisture.

This medication treats dyspeptic symptoms like bloating, heartburn, and delayed gastric emptying. It also addresses acute vomiting from migraines, infections, or medications. For Parkinson’s patients, it counters dopamine agonist-induced nausea. Clinical studies show 0.05% incidence of extrapyramidal reactions, making it safer than central dopamine antagonists.

Dosage guidelines: Adults take 10-20 mg (10-20 ml suspension) 15-30 minutes before meals. Pediatric doses are weight-adjusted (0.2-0.8 mg/kg). Maximum treatment duration for acute nausea is 12 weeks. Avoid with MAO inhibitors and opioid analgesics due to reduced efficacy.

Common side effects include dry mouth (1.9%), headaches (1.2%), and mild diarrhea (0.2%). Rarely, it may elevate prolactin levels, causing breast tenderness. Pregnant women should avoid it, but breastfeeding is safe due to minimal milk transfer.

Adults: 10-20 mg every 6-8 hours (10-20 ml suspension). Pediatric: 0.2-0.4 mg/kg every 8 hours. Administer 15-30 mins before meals and bedtime. Limit acute vomiting treatment to 12 weeks.

Side-effects are rare; exceptionally some transient intestinal cramps have been reported. Extrapyramidal phenomena are rare in young children and exceptional in adults: they reverse spontaneously and completely as soon as the treatment is stopped. As the pituitary gland is located outside the blood-brain barrier, domperidone may induce an increase in the plasma prolactin level. In rare case this hyperprolactinaemia may give rise to neuroendocrinological phenomena such as galactorrhoea and gynaecomastia. When the blood-brain barrier is immature (as in infants) or impaired, the possible occurrence of neurological side-effects cannot be totally excluded. Rare allergic reactions, such as rash and urticaria, have also been reported.

Pregnancy Category: Not approved. Lactation: Compatible with caution – minimal excretion in breast milk.

High extrapyramidal risk in infants <1 year. Hepatic/renal impairment requires dose adjustment. Monitor prolactin levels during prolonged use.

Pediatric: Caution due to immature BBB. Geriatric: Assess renal function. Hepatic impairment: Reduce dose. Renal failure: Extend dosing interval.

Symptoms of overdosage may include drowsiness, disorientation and extrapyramidal reactions, especially in children. In case of overdosage, the administration of activated charcoal, and close observation of the patients are recommended. Anticholinergic, antiparkinson drugs or antihistamines with anticholinergic properties may be helpful in controlling the extrapyramidal reactions.
